If you require immediate help all you need to do is complete the ‘Message Us’ form to the right of the page. A member of our admissions teams will call you back as soon as we receive your details.

If you’re a professional looking to refer someone to our services, please complete the application form below. Once we’ve received your referral you’ll be sent an email acknowledging receipt, a member of our admissions team will then contact you to arrange an assessment for your client.

Self-funded referrals

If you’re looking to self-fund your treatment, or if funding is being provided by a friend, family member or through an insurance policy please click the link below and complete the application form.
